Comparison of Intravenous Lidocaine versus Hyoscine on Prevention of Oculocardiac Reflex in Strabismus Surgery
نویسنده
چکیده
Purpose: Oculocardiac reflex (OCR) is a frequent event during strabismus surgery. The aim of this study is to compare the preventive effect of intravenous lidocaine and hyoscine on OCR. Methods: This prospective, randomized clinical trial was performed on 75 patients (5-63 years old) underwent strabismus surgery. Patients divided randomly into three groups. Intravenous hyoscine (0.2 mg/kg), lidocaine (1.5 mg/kg) and distilled water were injected before induction with propofol for each group respectively under standard monitoring. Heart rate, blood pressure and/or cardiac dysrrythmia were recorded immediately before/after injection and muscle hooking. Decrease of heart rate more than 20%, hypotension (decrease more than 20% in systolic or diastolic pressure) or any cardiac dysrrythmia were defined as OCR. Results: One hundred and thirty-six muscles were hooked during strabismus surgery. OCR was observed in 44 hooked muscles (32.3%). Reflex was observed in hyoscine (34.2%), lidocaine (24.9%) and control (40.9%) groups (p=0.12). The most stretched muscles which caused OCR were medial rectus (56.8%) and lateral rectus (36.3%) muscles. OCR was greater in patients with age between 5-10 years (56%). Conclusion: Intravenous lidocaine and N-buthylbromide hyoscine both could reduce OCR occurrence but the difference was not statistically significant compared to the control group. OCR was seen greater in young patients (5-10 years old). Some patients (specially children) need intervention to restore normal condition.

Sub-Tenon's injection versus paracetamol in pediatric strabismus surgery
BACKGROUND Emergence agitation, vomiting, and oculocardiac reflex (OCR) in children undergoing strabismus surgery under general anesthesia are common problems. The purpose of this study was to determine whether the effect of analgesia can reduce the incidence of these problems. We compared the effects of sub-Tenon's injection versus intravenous (IV) and rectal paracetamol in this surgery. MET...

Aim. Efficacy of granisetron and combination of granisetron and dexamethasone was evaluated for prevention of postoperative nausea and vomiting (PONV) in children undergoing elective strabismus surgery. Methods. A total of 136 children (1-15 years) were included. Children received either granisetron (40 mcg/kg) [group G] or combination of granisetron (40 mcg/kg) and dexamethasone (150 mcg/kg) [...

متن کاملDexmedetomidine Nasal Sedation Produces More Oculocardiac Reflex During Strabismus Surgery.
To the Editors: Dexmedetomidine (Precedex; Hospira, Inc., Lake Forest, IL) is an alpha-adrenergic agonist that can serve as an intranasal alternative to oral midazolam for children undergoing strabismus surgery. Helpful for pediatric eye surgery,1 when combined with fentanyl and ketamine, intravenous dexmedetomidine may decrease oculocardiac reflex.2 From 2013 to 2015, in our institutional revi...
متن کاملOculocardiac reflex as a predictive sign of postoperative nausea and vomiting following strabismus surgey in children
Background In this prospective, non-randomized study, we tested the hypothesis if oculocardiac reflex (OCR) can be used as a predictor of postoperative nausea and vomiting following strabismus surgery in children. Methods 88 children aged between 2 and 16 years undergoing strabismus surgery were enrolled in the study between 2009 and 2011. All anaesthetic drugs were given in a specific sequence...
